I just cropped out the status bar.  It shows all green as well.  I just opened the archive file that I attached to this post and simulated it to make sure there wasn't a difference between what I sent as well.

I assume that this is related.  The attached tool path should give a "Tool Flute isn't long enough" error and it doesn't.

I've found that I get collisions when I change the tool shoulder length to the flute length but not with the shoulder length larger than the cut depth.
